According to the DSS, "DEX SCORE Assessments help to tell the story of what has been achieved for a client / group / community as a result of service delivery".
SCORE stands for Standard Client/Community Outcomes Reporting. It forms part of the Data Exchange partnership approach.
An individual client SCORE assessment should be recorded at least twice, towards the beginning of service delivery and again towards the end.
Numerous SCORE assessments can be added if the client will be accessing the service long term. These SCORE's should be recorded using the same domain to measure the changes experienced by clients over time.
You do not need to record SCORE assessments against each domain listed; only those applicable to the client.
The Community component has been designed to report outcomes being achieved for large group where it is not feasible to record changes for individuals. Community SCORE's are recorded in a similar way to client SCORE's.
How to create a SCORE assessment in the All File Notes tab
- Click the Manage Clients button on the Toolbar
- Select the All File Notes tab
- Select the File Note for which the Score assessment is to be recorded
- Right-click on a file note and, in the context menu, select 'New DEX Assessment (SCORE)'. The New Assessment window will open.
- Select the 'Assessment by' option. For Ivo users, you will select one of the SCORE directly options from the top of the list.
- Tick the appropriate boxes for the selected domains. Remember, you will need to use the same domains that were selected in the pre-service assessments when recording the post-service assessments.
- When finished, 'Save and Close' to record the assessment.
